Case Manager QIDP (Qualified Intellectual Developmental Professional)
Responsible to develop and implement individual programs to maximize the client’s independent community living skills and inclusion in their community. Provide documentation on client in accordance with DHS, CARF, or DPH standards, as applicable. Advocate for and maintain rights and privacy of clients that are served.
Responsible for a caseload and is the designated team leader for the assigned area. The QIDP trains the DSP’s in the implementation of the Individual Service Plan. The QIDP is a role model and provides a positive, professional and safe environment in which to live, learn and work. Maintain and assist with assurance that all aspects of community living complies with all applicable Federal, state, and accreditation standards. Providing and participating in direct service is essential to team building.
AID facilitates services for Individuals from diverse backgrounds, embraces differences in individuals and promotes a positive attitude with peers and community members.
- Bachelor’s Degree in a human service field.
- At least one year of experience working directly with individuals with developmental disabilities.
